- A filled out and signed Cancellation and Refund Policy
- A bank statement that shows the minimum balance of $14,000.00 U.S. If there is a sponsor, the sponsor must prepare this document.
- A non-refundable application fee of $100.00 U.S. (do not send cash)
- If you plan to bring your dependent(s), a completed Dependent Information Form must be submitted with the complete application.
